If you have not been provided a username and password, you will not be able to file a claim through this portal. This Claim Form should be completed if you have experienced monetary and/or time losses as a direct result of The New London Hospital data incident that occurred on or about July 30, 2020, including out-of-pocket expenses, fraudulent charges, lost time spent dealing with fraudulent charges or unreimbursed extraordinary monetary losses.
