Welcome to the Mattress Firm (Bed Tech) Settlement Claims portal. If you received a Notice in the mail, and you have been provided with a unique identifier (Login and Password), you may file a Claim through this portal. If you have not been provided a Login and Password, you will not be able to file a claim through this portal. This Claim Form should be completed if you have purchased a Bed Tech-brand HR Platform bed frames bearing the Model Nos. HR33, HR33XL, HR40, HR50, HR60, and HR66 between April 8, 2015 and April 8, 2021.
A PDF of the Claim Form is located at the bottom of the Online Claim Form page. You will need to access the Online Claim Form using the Login and Password provided to locate the PDF of the Claim Form. The deadline to submit a Claim Form to participate in the Settlement is August 4, 2023.
