HUMANA-REIMBURSEMENT CLAIM FORM
Welcome to the Humana Claims portal. If you received notice regarding a Notice of Privacy Incident regarding the Data Incient from Humana Inc., you can file a Claim through this portal. This Claim Form should be filled out if you want to enroll in complimentary identity monitoring services through IDX for a period of two years from the Effective Date of the Settlement, receive a cash payment of (1) up to $250 for reimbursement for documented ordinary expenses and/or lost time that resulted from the Data Incident, and (2) up to $5,000 for reimbursement for documented extraordinary expenses.
